So what's the in-universe means that codenames get assigned? Do they let the operative choose his childhood nickname, let him pick from a list, or just assign it by fiat?

In the real-world, you NEVER get to pick your nickname. Those that do just get an eye-roll and tagged with something bad.

In an FTX before a deployment, we had a guy try to name himself. He was forever known as "Pillow Biter."

On my second deployment to Okinawa, one of our guys was trying to get everyone to call him "Killer." He was one of those stupid punk-arse kids who thought he'd get respect by talking about his jail time (which we later found out he never served). He went to an Okinawan tattoo parlor and got the word "Killer" tattooed across his back but in Japanese. What he got was not the Japanese character for the word "Killer," but the characters that symbolize the sounds made to make the English pronunciation of the word "Killer." The Tattoo artist left off a line on one of the characters, changing the way the sound is made, (like leaving off the bottom line on an uppercase "E" makes it an "F") this changed the way the word would be sounded out. We learned from the Barracks' Mama San what the word was and the closest approximation was "Kilpfer." Which we, as his peers, had to exploit, as Marines are wont to do, and he served out the rest of his enlistment with us know as "Klipper."
